Our files include your Social Security number, but we are missing your covered family members’ Social Security numbers. We are requesting that information from you. To do that, you can either (1) call 844.891.4602 toll-free (have your member ID number handy) or (2) download a Substitute W-9 Form using the Download link below.

The PDF file includes space for you to provide the Social Security numbers for seven covered individuals. If you need more space, please continue on a second sheet of paper.
