Maternal health benefits
G.E.H.A's maternal health coverage is designed to provide quality care for a safe, healthy birth, with guidance before, during and after delivery.
Doula coverage
G.E.H.A covers doula maternal support throughout pregnancy, labor/delivery and post-partum up to $1,000 per pregnancy (but not greater than the cost of doula support; deductible applies for HDHP members). Doulas must be certified to provide doula services to meet the Plan requirements of a covered provider as outlined in the plan brochure. Members are responsible for all charges in excess of $1,000.
Maternal mental health benefit coverage
G.E.H.A covers the first five visits per year, per pregnancy for office-based treatment of prenatal and postpartum depression with an in-network provider. Services include: diagnostic evaluation, medication evaluation and management, treatment and counseling. After the first five visits, benefits apply as outlined in the plan brochure.
Childbirth class benefits
Members receive up to a $150 allowance (but not greater than the cost of the class; deductible applies for HDHP members) for childbirth classes, both in-network or out-of-network, as long as they are offered through a covered provider or facility.
Breast pump benefits
How to order your breast pump
Your breast pump benefit covers one dual suction, double-channel electric breast pump at 100%, with a physician's prescription, every 12 months when purchased from an in-network provider (such as your OB/GYN). A new prescription is required when requesting a new or replacement pump. Additional pumps may be obtained at your expense. Limit one pump every 12 months.
When to order your pump
Members can order as early as 30 days prior to their due date or once baby is born if they deliver prematurely.
Where to get your pump
You may obtain the breast pump and supplies from a contracted (in-network) provider (such as your OB/GYN). Not all providers are contracted to supply breast pumps (or associated supplies), so confirm with you provider if they are contracted with the UnitedHealthcare Choice Plus network to provide breast pumps. A list of nationwide providers is included below — these providers can ship on a nationwide basis. Providers may have different brands or models in stock, which are subject to change.
If you choose to use a non-contracted (out-of-network) provider, you will be responsible for costs as outlined in the plan brochure. You will not be reimbursed for a pump or supplies purchased at a retail store.
