When GEHA has been notified of your enrollment, you will receive an enrollment kit that includes a plan brochure and an Enrollment Questionnaire. To save time and help speed up access to your benefits, you can complete and submit the online Enrollment Questionnaire by clicking "Fill in Form Online" below (instead of mailing in the form you received in the mail).
