A personal health assessment is a confidential, online survey that helps identify lifestyle habits that can lead to potential illness. It is a way you can learn what you’re doing right and what you need to work on to be healthier.
The questions are related to lifestyle practices and health history factors that have the highest impact on your health, and biometric measures to determine health status. Lifestyle questions focus on exercise, nutrition, smoking, alcohol use, stress and well-being. Biometric measures are things like height, weight, blood pressure and cholesterol.
GEHA offers a health risk assessment as part of our Health Rewards program for members. It is a good idea to take the assessment every year. It is easy to take and after completing it, you will receive an instant personal wellness report that includes your “good habits” score and health strengths. It will also identify possible health risks so you know where to make any lifestyle changes.
In addition, after completing the assessment you will receive a $75 Health Rewards prepaid MasterCard, which can be used to pay for qualified medical expenses or health-related items from the online FSA store.
