Here's how get started with your new medical plan:

1. Watch your mailbox for your ID cards.

If you haven't already, you will soon receive your plan ID cards. In separate mailings, you will receive:

  • A set of health plan ID cards with the name of the member eligible for the medical plan through FEHB. If you are the member, your spouse's and dependent's cards will have your name on it. Plan ID cards are also used at the pharmacy as your prescription drug card.
  • A vision ID card.

2. Fill out your Enrollment Questionnaire.

You'll receive a packet of plan materials from GEHA that includes a yellow Enrollment Questionnaire. This form helps us process your claims accurately and quickly. Please fill it out at your earliest convenience. If you have misplaced that yellow form, you can also complete the Enrollment Questionnaire online.

3. Take advantage of wellness programs and other benefits.
